HHS Requests Public Input on Vaccine Frameworks
The Department of Health and Human Services has opened a public comment period to review how federal vaccine recommendations are categorized and communicated, including whether the current three-tier framework should be expanded or revised. The move comes under President Trump’s executive order to stagger or reduce childhood vaccinations, with an emphasis on timing, sequencing, and the potential expansion of categories beyond routine, risk-based, and shared clinical decision-making. HHS is seeking input on whether shared decision-making should be broadened or replaced with terms like conditional recommendations, and how these categories affect access, understanding, and trust in public health guidance. The effort follows actions by health and pediatric groups earlier this year to space out vaccines, a shift that has been met with pushback from several states and the American Academy of Pediatrics. Public input is also intended to inform how evidence strength, variability among individuals, and practical communication should shape recommendations and maintain public confidence in immunization programs. The initiative is being coordinated with the Task Force on Safer Childhood Vaccines to ensure science-based, transparent processes in vaccine policy.
