Cranberry juice's reputation for urinary tract health comes down to compounds called proanthocyanidins (PACs), a type of tannin found in cranberries. The leading theory is that these PACs interfere with the ability of E. coli bacteria—the cause of most urinary tract infections (UTIs)—to attach to the cells lining the bladder and urinary tract walls. Bacteria typically use small hair-like structures called fimbriae to latch onto the urinary tract lining before multiplying and causing infection. Cranberry PACs are thought to bind to these fimbriae or otherwise coat the bladder wall, making it harder for bacteria to stick around long enough to establish an infection. If bacteria can't adhere, they're more easily flushed out through normal urination.
An older, now largely discredited theory held that cranberry juice worked by acidifying the urine, creating an environment hostile to bacteria. Research has since shown that typical consumption doesn't lower urine pH enough to meaningfully affect bacterial growth, so the anti-adhesion mechanism is considered the more plausible explanation.
It's important to note that scientific evidence on cranberry juice's effectiveness is mixed. Some clinical trials and reviews (including analyses by the Cochrane Collaboration) suggest cranberry products—particularly concentrated capsules or supplements with standardized PAC content—may modestly reduce recurrent UTI frequency in certain groups, such as women with a history of frequent UTIs. However, many commercial cranberry juices contain low concentrations of PACs and high amounts of added sugar, which may limit their benefit compared to concentrated extracts. Cranberry juice is also not considered a treatment for an active infection; it's viewed more as a possible preventive measure.
