The primary causes include inadequate insulin secretion, insulin resistance, and impaired glucose utilization—all stemming from incomplete development of the pancreas and metabolic organs.
Premature babies face several specific risk factors. Stress from illness, respiratory distress, or infection triggers the release of stress hormones (cortisol and catecholamines) that raise blood glucose levels. Intravenous nutrition, particularly glucose-containing fluids given to support growth, can deliver more glucose than the infant's pancreas can handle. Additionally, immature liver function reduces the organ's ability to store and regulate glucose effectively.
Infections are a common trigger, as sepsis and other infections increase insulin resistance. Medications such as corticosteroids—sometimes given to prevent respiratory complications—also raise blood sugar. Certain conditions like necrotizing enterocolitis (bowel inflammation) disrupt normal metabolism further.
Temperature instability and inadequate nutrition can exacerbate glucose dysregulation. Unlike older infants and children, premature babies have limited glycogen stores and cannot maintain stable blood sugar during fasting periods.
Hyperglycemia in premature infants requires careful monitoring because persistently elevated blood sugar may impair healing and increase infection risk. Treatment typically involves adjusting the rate and composition of intravenous fluids, ensuring adequate but not excessive glucose delivery, and addressing underlying infections or complications.
