Fetal sex can sometimes be glimpsed on ultrasound as early as 12 to 14 weeks of pregnancy, but accuracy at this stage is limited — the genital tubercle (the tissue that develops into either a penis or clitoris) hasn't fully differentiated, and the angle it points can be misread. Sonographers who attempt a prediction this early typically look at the "nub theory" (the angle of the genital tubercle relative to the spine), which some studies suggest can be around 70–75% accurate as early as 11–13 weeks, improving as the pregnancy progresses.
For a much more reliable read, most providers wait until the anatomy scan, usually done between 18 and 22 weeks. By this point the external genitalia are clearly formed, and accuracy for an experienced sonographer is generally cited in the 95–99% range under good conditions. Even then, results aren't guaranteed — fetal positioning (legs crossed, baby facing away), low amniotic fluid, maternal body habitus, or an early gestational age can all obscure the view and lead to an inconclusive or occasionally incorrect reading.
If you want an earlier and more definitive answer than ultrasound can offer, options include:
- Non-invasive prenatal testing (NIPT), a blood test that analyzes fetal DNA circulating in the mother's blood, which can determine sex with very high accuracy as early as 9 to 10 weeks.
- Chorionic villus sampling (CVS), an invasive diagnostic test done around 10–13 weeks, which also reveals sex but carries a small miscarriage risk and is usually reserved for genetic screening purposes rather than sex determination alone.
Keep in mind that ultrasound-based sex prediction is not the primary purpose of these scans — they're mainly performed to check fetal growth, anatomy, and health. Because early guesses can be wrong, many parents choose to confirm findings at a later scan or via blood testing before making firm plans, and it's worth discussing timing and accuracy expectations with your own healthcare provider.
